Unwanted Materials Removal
In order to request an unwanted materials removal, the generator must populate an Unwanted Materials Acceptance Form (UMAF) online through various drop-down menus (see instructions below). Once the UMAF is submitted, EH&S personnel will know exactly what to look for in the labs. The completed UMAF can also be saved for future requests. Waste generators must still fill out the green waste tags and enter waste information into the new system.
If a generator has 70 or more chemicals do not complete the UMAF at this time. Please contact EH&S at 294-5359 for assistance.

Instructions for filling out the Unwanted Materials Acceptance Form
Please Note: All fields are required for processing of the form. Please do not leave any fields blank.
- Fill in the requestor’s ISUID number. If you do not have an ISUID please contact EH&S at 294-5359.
- Fill in the department, building and room from the various drop-down menus. Once those three fields are completed the Satellite Accumulation Area number (SAA#) associated with your area will fill automatically. If you know your SAA# then you only need to enter it and the department, building and room fields will fill automatically. Write down your SAA# for future reference.
- Provide the information for the material needed to be picked up; size of the container, how many containers and the material description. You may either select a material description from the drop-down or type in your own description. Check the radioactive box if the material is radioactive.
- Select the “Add Material” box for additional items and repeat the step above.
- When finished select “Submit Request” to send request to EH&S. The unwanted materials must be located in the proper SAA, tagged and labeled to correspond with the UMAF. You should expect an unwanted materials collection within 1-3 business days, so please have the materials ready for collection.
- There is no need to print out your request submittal or fill any paperwork at all and leave with the unwanted materials, EH&S personnel will have the paperwork necessary for the pick up.
