Project Approval

The individual responsible for the project (Principal Investigator or PI) completes the "Application for Use of Radioactive Materials, Devices and Lasers" form. This form may be obtained from EH&S (294-5359) as a printed copy or downloaded using the link above. If the project involves the use of live animals, it also requires the approval of the Committee on Animal Care (COAC) which can be initiated by submitting a complete. COAC Protocol Review Form.

The completed radioactive material or device application is submitted to the Radiation Safety Officer, EH&S, 2809 Daley Drive, where it will receive an initial review. If the application is complete, the Radiation Safety Officer will forward it to the Radiation Safety Committee (RSC) Chair, who will distribute copies to the other RSC members if necessary. After reviewing the application, the RSC Chair will decide whether to grant authorization. If the application is approved, the signed authorization form will be returned to the PI. Any special conditions under which authorization is granted will be specified on the authorization form.

If approval is not granted, a written notification will be sent to the PI including an explanation of the decision.

For changes to a current Radioactive Material Use Authorization, the Principal Investigator must send the request to the Radiation Safety Officer.
